I hope patients are being routinely screened using the PSQ (Appendix 3 in Ryle and Kerr 2002). Those who score 28+ or are otherwise identified as borderline need diagrams identifying their different self-states (ie partially dissociated rrps). These need to be as accurate as possible. With help from Dawn Bennett I have developed a procedure - the SDP - which patients can carry out - usually with a little support.
Part 1 offers descriptions and provisional names for a range of common borderline states; patients recognise which of these they experience. Part 2 elicits detailed data about the frequency, duration, causes of onset and exiting, associated physical and psychological symptoms etc for each of the recognised states - a copy of this part 2 page is completed for each state.
CLINICAL APPLICATIONS
This procedure yields more complete and systematic information than is usually obtained. I believe it would be clinically useful for therapists at any stage of training who are reformulating and treating borderline patients. The descriptions offered, from which patients identify those that apply to each state, are in a form which indicates the reciprocal and so allows self-states to be identified and described by the therapist.
RESEARCH USE
By collecting completed Part 2s from a large number of patients we can assemble profiles of the commonly encountered states and develop a CAT- based account of the phenomenology of BPD.
